I've just seen some overhead photos of the BN version of these trains where there's a cab car also included "mid-train". In fact, the trains could be pretty easily produced by buying both Rapido sets--has a certain appeal to me. Here's the best example I found: https://www.railpictures.net/photo/383282/ You can see the horns on the middle car and the rear one. Did the Q do this, also? Why was it done?
