I know that a repeated concern by the US DOT and the NTSB for years and years has been the lack of standarization. The oldest example I can think of is the Accident Investigation of an IC/BRC collision at the Harvey Interlocking in Markham when the BRC crew had no way of communicating with the Harvey Switchtender or any other IC personnel (their radio was on a different frequency and the lineside phones were dead and had been for years), and they didn't have IC Special instructions or employee timetable. The BRC crew mis-interpreted a very strange signal on the IC and ended up running a red board. More recently the NTSB was aghast at the NS/Amtrak collision a couple years back when they considered the number of different sets of signal aspects that crews in the Chicago area had to understand..
