Now we know there has long been a rule on RR's that doors must be closed on box cars before moving. Some RR's, past & present, enforced this on shippers; others did/do not. Back in late 70's the President of a southern short line was killed while standing trackside @ hdq to give one of his passing trains a "roll-by". Door was open and chose the exact moment car passed him to come off tracks (the guide at top of door) to fall atop him. If you ever tried to left a boxcar door you know how heavy they are! Back in the days when box cars were prevalent, it was not uncommon to see one every mile or two along ROW of lesser tracks (i.e. bumpier). I still have that particular ICC report from my Safety Mgr days @ C&NW.
