Ted,
I’m surprised they put a paper sheet on top to protect the pad, I have a bundle 
of Milwaukee train order pads still in the shrink wrap and I don’t recall the 
thicker paper on those, but then it’s been a while since I looked at them.

The Form 19 train order form from the Burlington I have was found in a box of 
folders at a model railroad club, it may have faded before it was folded up in 
the folder.

No clearance was with it and I don’t see staple holes in it so it may have been 
a file copy.

Don’t know how well the photo is below.
