All - Some sharp eyes here! You all are correct, the "reach rod" connected to the power reverse is badly bent.  Simply put, the rod between the power reverse linkage and the linkage on the hanger mounted valve gear should be horizontal, not u-shaped.  
 
I'll speculate that the failure happened in the engine servicing area as the tender is heaped high with coal and the S-4-A otherwise appears to be serviceable. The bent reach rod may've proved fatal to 4004 as the Corbin book reports that the 4-6-4 was the first of its class to be sold for scrap in May 1955.

Group - Here's an old undated image (inserted and attached) of Class S-4-A No. 4004 taken at the Clyde Engine Terminal in the Cicero, IL, yards. Question is, what's "wrong" with the locomotive? For what reason is it awaiting repairs?
