I may add that one of the most obvious differences was
the difference in door height. Older meat reefers with
swinging doors usually had shorter vertical door openings
than your average fan type.
The ART 9xxxx series of meat reefers is an easy to see
example of this design alteration. Many meat reefers
had a flat plate above the doors. This was not done with
cars intended for fruit or vegetables.

And yet that isn't such a flip answer. One real difference, aside
from the mentioned ventilation issue, is the ability and need to
control the temperature through a variety of ranges (for produce) or
maintaining it within one fairly constant range (meat). The ability
to ventilate or not also includes the variations on the hatches on
top (typically) of the car. Another difference includes the loading
(aside from the hooks and rails) and the ability to blow ice
throughout the car to cool the entire load as well as having the ice
in the bunkers. Include the provisions for the dispersion of salt as
well.
