Per current General Code of Operating rules...
(Paraphrase)
Headlights must burn at full brightness (including
ditchlights) when trains are occupying main track
except...
When meeting or passing trains or switching in the
yard, light must be dimmed
Also,
When train is tied down in the clear of the main track
headlight must be extinguished.
THAT SAID...
Early in the steam era, the rules read that the
headlight burn only at night being considered a "night
time signal."
Therefore, most daylight trains didn't run with
headlights burning.
At night the rules regarding dimming did apply in very
similar ways that they do now.
Not sure when the rules changed.
