I have to agree that the most likely way to get an engine like this is to have it custom built.  Roger Miller built the Prairie below, and later sold it to Jerry Lundeen.  Roger had many brass casting made specifically for this model and others, including the dome, the freight pilot, the bicycle trailing truck gear, and other parts.  He did, indeed, start with the Balboa AT&SF 2-6-2 for this model.

I purchased this from Jerry several years ago, and had it sent out for DCC, sound, and directional lighting.  It's numbered 2151 and it's serving as the switch engine for Oregon, IL and Mt Morris, IL to switch both the town, and more importantly, the Kable Brothers printing plant.  In Zephyr #47 (available to members on the website) Jim Singer's excellent article on Oregon and Mt Morris covers these two towns well.  Steve Holding also had excellent coverage of Kable Brothers printing in Zephyr #67.

Because there was no way to turn the Oregon switcher, the steam engine had to run around the train, and with it's nose facing eastward, pull the train up to Mt Morris backwards.  I'm currently working on the scenery for Mt Morris on my December 1950 layout.

Another option would be to find a custom builder that can build from scratch or convert an existing model over to a R4 or R5.  The only Burlington Route R4 or R5 I know of being converted was done using a Santa Fe #1800 class Prairie.  These models were imported by Custom Brass and also Balboa I believe.  A new tender would have to be made or possibly converted from an existing Great Norther roll top style tender.  New domes would have to be sourced or made and mounted to the top of the boiler.  There may be other existing brass Prairies that match closely in measurements with an R4 or R5 that could be used as a start.  From looking at photos it looks like the Prairie boilers are a simple cylinder with no angles and humps / slopes like an O1a 2-8-2.  The drivers are 69" tall... one might be able to simply mount a cylinder on an old MDC / Roundhouse 2-6-2 drive and start from there.  Don't forget that Michelangelo's statue of David started off as a huge chunk of marble and he simply started carving off everything that didn't look like David!
