Having been involved with the Heavy-Haul truck industry, I can offer some comments on your question.  Such as:

 

Maybe one issue is tunnels.  There are routes on the western roads that offer travel for great distances without a tunnel or overpass.  For example, there is a route from Los Angeles harbor toward Salt Lake City without any overpasses.   Some of the roads are unpaved and on private or government property, but the haul can be done.

 

Maybe the railroads don’t want the hassle of moving stuff that large.  If I’m moving 108 cars of coal or 300 containers, the railroads are happy to talk.  But if it affects operations a bit, or delays one of those 108-car coal train, maybe the response is less than cordial.  And there is a lot of competition among the heavy haulers, bring costs down.
