Every once in a while this rumor or story comes up. Usually rumors are rooted in truth but I don't think anyone has ever come up with actual proof that the Burlington Route had an O5 4-8-4 that they converted to a six wheeled trailing truck. There may have been some thought of doing this though in the motive power department and it has filtered down over the years. There's also a story that the Burlington may have considered taking the M4a 2-10-4s and removing the boilers from the drive and slapping the superstructure down onto a 4-8-4's drive. Presumable this would have been to make a faster locomotive than the 50mph or so M4a 2-10-4.

The Burlington Route did however take a 2-8-2 and make a "heavy switcher" out of it by removing the lead pilot truck. It didn't work out to well and apparently may have derailed a bunch so that experiment wasn't repeated. It's number, I think, was #5020 and the locomotive started out as an O1 or O1a class 2-8-2.

A friend started chatting with me about a supposed CB&Q 4-8-6 steam locomotive. So I went out and checked the Q roster and did not find any 4-8-6’s listed at all. I then went out to see if any such wheel arrangements were ever built. 

Turns out that Lima had proposed one but none were built. But then I found out that in 1950, the Q experimented using 5601 (a 4-8-4) putting the 6 wheel truck under the cab to test on branch lines. The test was a failure after less then a year.
 
Would be interesting to see photos of 5601 during that time.
