If it's any help, the roster in the back of the Corbin/Kerka book on page 285 shows what I interpret as Extra Order 5356-35 for Class S-4 No. 3012 whose boiler was fabricated by Baldwin in 1935.
 
Do you or anyone else on this List know why the Q opted for a Baldwin boiler for the 3012 after having constructed the others themselves at West Burlington for preceding S-4 locomotives? 
 
The Q must've been happy with quality and price of the out-sourced boiler as they went back two years later in 1937 to Baldwin for the series of O-5-A boilers.
 
Perhaps, the S-4 boiler was a "try out" for Baldwin since the O-5-A design work was well under way by that time. Evidently, Baldwin could build the boilers faster and for less money than the Q could.
 
Always enjoyable to speculate at this late date.

6675 isn’t a regular “builder’s number” unless Baldwin started the numbering again. 6675 would have been allocated in about 1883!

Hol Wagner, reference the West Burlington Shops image of the 5626, recently commented:

 

The above plate was removed from an O-5-A in the Galesburg Dead Line for my father who wanted one for his collection. I do not recall what locomotive it was removed from. Perhaps, someone on this List can identify the 4-8-4 from the plate's construction number.
