Regarding efficiency tests, not sure about ‘back then,’ but now days, when the BNSF folks go out to test passenger train crews (Amtrak), they have to have an Amtrak road foreman with them.  That way, if the crew fails, and it is a fatal test (getting by the red signal), the crew can be taken out of service on the spot, and the RFE brings the train in.

 

Guess how they came to that testing plan?  Maybe Steve knows the rest of the story.  All I know is that somebody flunked and nobody could move the train until after they called a new crew and got them out to the train, hours later.
